Labiaplasty Potential Risks

No surgeries are performed without risks. Dr. Vu’s goal is to perform the planned procedure, take extra precaution during surgery, and most important of all is to be safe and yet achieve utmost optimal outcomes. Following are some of the potential risks that are commonly discussed about regarding labiaplasty surgery.

A. Incision (scar)

The incisions will vary depending on the modification or correction required. Most scars will fade over time.

B. Alteration of sensation

Surgery does carry the risk of alteration of sensation. However, with this procedure it is very rare.

C. Asymmetry

Minimal asymmetry of the labia can occur, but is usually not cosmetically significant.

D. Skin Loss

Skin loss may occur from infection or damage to the blood supply. However, the excellent blood supply of this area reduces this risk.

Labiaplasty Postoperative Care

Special attentions are placed during your postoperative period. Dr. Vu’s goal is to make sure that you are comfortable after you leave the surgery center, and have proper instructions how to take care of yourself in order for you to have a pleasant surgical experience.

A. Immediate Labiaplasty Post-op Instructions

Immediately following Labiaplasty you should remain in bed, laying as flat as possible, with an ice pack for comfort. This will markedly reduce the amount of post-operative swelling and discomfort. It is normal to have swelling and bruising of the labia. This will resolve over the next 2 weeks.

B. Dressings

The surgical dressings can be removed anytime they are saturated and then replaced with a sanitary napkin or maxipad. There will be bloody or blood-tinged drainage for a minimum of 1-2 weeks following a Labiaplasty. Bacitracin antibiotic ointment should be applied to the incisions twice a day for 14 days. Please apply a thin coating only. Excessive amounts of ointment are not necessary. You may switch to an Aloe Vera ointment after 2 weeks.

C. Activities

There will be relatively minor discomfort with the Labiaplasty for most patients. However, some patients do experience moderate to severe swelling that can cause much discomfort. Diligent post-operative application of ice pack may reduce swelling thereby reducing pain. Patients who experience minimal swelling and bleeding should be able to resume sedentary work activities within a few days, resume light exercise at one week, and have no restrictions after two weeks. However, those with more excessive swelling and discomfort may want to limit their activities until they feel comfortable and gradually progress to their normal activities.

D. Sexual Activities

As a general rule, you may resume sexual activity 6 to 12 weeks after surgery. This is totally patient dependent upon how extensive the surgery was you had done. Dr. Vu will let you know when you are sufficiently healed.

E. Swelling and Bruising

Immediately following a Labioplasty you should remain in bed with an ice pack on the surgical site for the first 24 hours. This will markedly reduce the amount of post-operative swelling and discomfort. It is normal to have swelling and bruising of the labia. This will resolve over the next 2 weeks.

F. Healing of sensory nerves

The skin of the labia may be partially numb for several months while the wounds are healing. Permanent numbness is very rare.

Women are multiorgasmic. The female sexual response cycle is unimpeded by a required refractory period that prevents successive orgasms in the male.

Principal understanding of the physiology of the human sexual response emanates from observational research studies by William H. Masters, M.D. and Virginia E. Johnson. The researchers tested 382 women and 312 men in more than 10,000 episodes of sexual activity. Dr. Vu’s goal is to acquaint you with the various forms of pelvic relaxation as well as their causes, symptoms, and treatment. The pelvic organs include the vagina, uterus, bladder, and rectum. These organs are held in position by three types of supports: 1) muscles, 2) sheets of tissue called fascia 3) and ligaments. When these supports become damaged for various reasons, one or more of the pelvic organs may sag and, occasionally, even protrude outside the vagina. These are called pelvic support defects.

Occasionally, this weakening of the muscles and tissue may occur in women who have never had children. In these women, the cause may be:

• Inherited weakness of the supporting tissues
• Unusual strain placed on the supporting tissues by a chronic cough
• Unusual increases in abdominal pressure
• Obesity

The general symptoms associated with pelvic relaxation depend on which organs are affected. Often there is a feeling of heaviness or fullness. Small or moderate amounts of urine may be lost with normal physical activities such as laughing, coughing, walking, or running. In more advanced and rare cases a mass may actually protrude from the vaginal opening. Based on the organ or organs involved, pelvic support defects can be defined more specifically as:

• Cystocele
• Urethrocele (most of the time the cystocele and urethrocele occur) in combination – cystourethrocele
• Rectocele
• Enterocele
• Uterine prolapse

A cystocele occurs when the bladder falls or descends from its normal position. The most common symptom associated with cystocele is difficulty in completely emptying the bladder. This can be associated with bladder infections. Large cystoceles can cause the bladder to overfill and allow small amounts of urine to leak. Leakage is most common during activity such as walking or bouts of coughing.

A urethrocele usually occurs in conjunction with a cystocele. Both of these conditions result in, among other things, involuntary loss of urine, particularly when there is increased pressure in the abdomen, caused by walking, jumping, coughing, sneezing, laughing, or sudden movements. Rectoceles happen when the rectum bulges into or out of the vagina.

Rectoceles usually occur as a result of injuries sustained during childbirth. With a weakened or bulging rectum, bowel movements become more difficult.

An enterocele is the bulging of small intestines into the back wall of the vagina.

Uterine prolapse occurs when the uterus falls or is displaced from its normal position. There are varying degrees of severity depending on the descent. This produces a general felling of heaviness and fullness, or a sense that the uterus is falling out.

The diagnosis of these problems includes a thorough history and physical examination. Other tests, depending on the circumstances, include a “Q-tip” test, urodynamic studies (a painless fifteen to twenty minute computerized bladder and urethra functional studies), urethrocystoscopy (instrument used to evaluate the inside of the bladder and urethra), X-rays of the urinary system.

For all practical purposes, definitive treatment is surgical correction of the specific defects.

Treatment
Your protected health information will be used by us for providing, coordinating
or managing your health care and any related services. This includes the
coordination or management of your health care with a third party that has
already obtained your permission to have access to your protected health
information. For example, we would disclose your protected health information,
as necessary, to a recovery facility that provides care to you. We will also
disclose protected health information to other physicians who may be treating
you when we have the necessary permission from you to disclose your protected
health information. For example, your protected health information may be
provided to a physician to whom you have been referred, to ensure that the
physician has the necessary information to diagnose or treat you.
In addition, we may disclose your protected health information from time-to-time
to another physician or health care provider (e.g., a specialist or laboratory) who,

Payment
Your protected health information will be used, as needed, to obtain payment for
your health care services. This may include certain activities that your health

necessity, and undertaking utilization review activities. For example, obtaining

support the business activities of your physician’s practice. These activities
include, but are not limited to, quality assessment activities, employee review
activities, training of medical students, licensing, marketing and fundraising

For example, we may disclose your protected health information to medical
school students that see patients at our office. In addition, we may use a sign-in
sheet at the registration desk where you will be asked to sign your name and
indicate your physician. We may also call you by name in the waiting room when
Dr. Vu is ready to see you. We may use or disclose your protected health information, as necessary, to contact you to remind you of your appointment.

We will share your protected health information with third party “business
associates” that perform various activities (e.g., billing, transcription services) for
the practice. Whenever an arrangement between our office and a business
associate involves the use or disclosure of your protected health information, we
will have a written contract that contains terms that will protect the privacy of
your protected health information.

We may use or disclose your protected health information, as necessary, to
provide you with information about treatment alternatives or other health-related
benefits and services that may be of interest to you. We may also use and
disclose your protected health information for other marketing activities. For
example, your name and address may be used to send you a newsletter about
our practice and the services we offer. We may also send you information about
products or services that we believe may be beneficial to you. You may contact

We may use or disclose your demographic information and the dates that you
received treatment from your physician, as necessary, in order to contact you for

BASED UPON YOUR WRITTEN AUTHORIZATION

only with your written authorization, unless otherwise permitted or required by
law as described below. You may revoke this authorization, at any time, in
writing, except to the extent that your physician or the physician’s practice has
taken an action in reliance on the use or disclosure indicated in the authorization.

BE MADE WITH YOUR AUTHORIZATION OR OPPORTUNITY TO OBJECT
We may use and disclose your protected health information in the following
instances. You have the opportunity to agree or object to the use or disclosure of
all or part of your protected health information. If you are not present or able to
agree or object to the use or disclosure of the protected health information, then
Dr. Vu may, using professional judgement, determine whether
the disclosure is in your best interest. In this case, only the protected health
information that is relevant to your health care will be disclosed.

Others Involved in Your Healthcare
Unless you object, we may disclose your protected health information to a
member of your family, a relative, a close friend or any other person you identify
when it directly relates to that person’s involvement in your health care. If you
are unable to agree or object to such a disclosure, we may disclose such
information as necessary if we determine that it is in your best interest based on
our professional judgment. We may use or disclose protected health information
to notify or assist in notifying a family member, personal representative or any
other person that is responsible for your care of your location, general condition
or death. Finally, we may use or disclose your protected health information to an
authorized public or private entity to assist in disaster relief efforts and to
coordinate uses and disclosures to family or other individuals involved in your
health care.

Emergencies
We may use or disclose your protected health information in an emergency
treatment situation. If Dr. Vu or another physician in the
practice is required by law to treat you, he or she may still use or disclose your
protected health information.

OTHER PERMITTED AND REQUIRED USES AND DISCLOSURES THAT MAY BE MADE WITHOUT YOUR AUTHORIZATION OR OPPORTUNITY TO OBJECT

We may use or disclose your protected health information in the following situations without your authorization. These situations include:

Required By Law
We may use or disclose your protected health information to the extent that the
use or disclosure is required by law. The use or disclosure will be made in
compliance with the law and will be limited to the relevant requirements of the
law. You will be notified, as required by law, of any such uses or disclosures.

Public Health
We may disclose your protected health information for public health activities
and purposes to a public health authority that is permitted by law to collect or
receive the information. The disclosure will be made for the purpose of
controlling disease, injury or disability. We may also disclose your protected
health information, if directed by the public health authority, to a foreign
government agency that is collaborating with the public health authority.

Communicable Diseases
We may disclose your protected health information, if authorized by law, to a
person who may have been exposed to a communicable disease or may
otherwise be at risk of contracting or spreading the disease or condition.

Health Oversight
We may disclose protected health information to a health oversight agency for
activities authorized by law, such as audits, investigations, and inspections.
Oversight agencies seeking this information include government agencies that
oversee the health care system, government benefit programs, other
government regulatory programs and civil rights laws.

Abuse or Neglect
We may disclose your protected health information to a public health authority
that is authorized by law to receive reports of child abuse or neglect. In addition,
we may disclose your protected health information if we believe that you have
been a victim of abuse, neglect or domestic violence to the governmental entity
or agency authorized to receive such information. In this case, the disclosure will
be made consistent with the requirements of applicable federal and state laws.

Food and Drug Administration
We may disclose your protected health information to a person or company
required by the Food and Drug Administration to report adverse events, product
defects or problems, biologic product deviations, track products; to enable
product recalls; to make repairs or replacements, or to conduct post marketing
surveillance, as required.

Legal Proceedings
We may disclose protected health information in the course of any judicial or
administrative proceeding, in response to an order of a court or administrative
tribunal (to the extent such disclosure is expressly authorized), in certain
conditions in response to a subpoena, discovery request or other lawful process.

Law Enforcement
We may also disclose protected health information, for law enforcement
purposes, so long as applicable legal requirements are met. These law
enforcement purposes include legal processes and otherwise required by law,
limited information requests for identification and location purposes, pertaining to
victims of a crime, suspicion that death has occurred as a result of criminal
conduct, in the event that a crime occurs on the premises of the practice, and
medical emergency (other than on the Practice’s premises) and it is likely that a
crime has occurred.

Coroners, Funeral Directors, and Organ Donation
We may disclose protected health information to a coroner or medical examiner
for identification purposes, determining cause of death or for the coroner or
medical examiner to perform other duties authorized by law. We may also
disclose protected health information to a funeral director, as authorized by law,
in order to permit the funeral director to carry out their duties. We may disclose
such information in reasonable anticipation of death. Protected health
information may be used and disclosed for cadaveric organ, eye or tissue
donation purposes.

Research
We may disclose your protected health information to researchers when their
research has been approved by an institutional review board that has reviewed
the research proposal and established protocols to ensure the privacy of your
protected health information.

Criminal Activity
Consistent with applicable federal and state laws, we may disclose your
protected health information, if we believe that the use or disclosure is necessary
to prevent or lessen a serious and imminent threat to the health or safety of a
person or the public. We may also disclose protected health information if it is
necessary for law enforcement authorities to identify or apprehend an individual.

Military Activity and National Security
When the appropriate conditions apply, we may use or disclose protected health
information of individuals who are Armed Forces personnel for activities deemed
necessary by appropriate military command authorities; for the purpose of a
determination by the Department of Veterans Affairs of your eligibility for
benefits, or to foreign military authority if you are a member of that foreign
military services. We may also disclose your protected health information to
authorized federal officials for conducting national security and intelligence
activities, including for the provision of protective services to the President or
others legally authorized.

Workers’ Compensation
Your protected health information may be disclosed by us as authorized to
comply with workers’ compensation laws and other similar legally-established
programs.

Inmates
We may use or disclose your protected health information if you are an inmate
of a correctional facility and your physician created or received your protected
health information in the course of providing care to you.

Required Uses and Disclosures
Under the law, we must make disclosures to you and when required by the
Secretary of the Department of Health and Human Services to investigate or
determine our compliance with the requirements of Section 164.500 et. seq.

About Dr. Kim-Chi Vu, Female Plastic Surgeon, Portland, OR

One of only a handful of female plastic surgeons in Portland, Dr. Kim-Chi Vu attended the University of Portland as an undergraduate and graduated from medical school at the Oregon Health Sciences University, where she also completed her surgical residency.

Dr. Vu, certified by the American Board of Plastic Surgery, specializes in not only cosmetic and reconstructions, but also in hand trauma surgeries. During her residency, Dr. Vu was involved in several research projects, including the heparin effect on thrombocytopenia, bioresorbable ribs stabilization in rib fractures, and a clinical study on composite grafting for correction of bilateral cleft nasal deformity.

In the ongoing pursuit of excellence in her field, Dr. Vu studied under the famed Dr. Matlock of The Laser Vaginal Institute of Los Angeles. Dr. Matlock is the recognized leader in the field of female genitalia surgery and is reponsible for developing the Laser Vaginal Rejuvenation and Designer Laser Vaginoplasty procedures that are seeing such rapid growth among women. Dr. Vu complete her advanced training with Dr. Matlock and was certified in August 2009.

Dr. Vu is the only plastic surgeon in Portland certified to use the latest laser techniques in Laser Vaginal Rejuvenation and Designer Laser Vaginoplasty surgeries for vaginal tightening and labiaplasty for the surgical correction of vaginal relaxation (loose vagina) and large labia or enlarged vaginal lips.

Originally from Vietnam, Dr. Vu and her family immigrated to the United States when she was about six years old. They originally resided in Tennessee and moved to Portland, in 1978, where she has remained. Dr. Vu is married to Andrew Nguyen, an engineer, and they have a seven year old son and a four year old daughter. Her hobbies include running, tennis, volleyball, and spending as much time as possible with her family.

Dr. Vu’s exceptional knowledge of her field comes not only from her plastic surgical experiences but also from continuing education by attending local as well as national meetings in the field of plastic surgery year around. In addition, she has a background in hand trauma, which is a valuable complement to her usual work. While very different in some respects from carefully choreographed cosmetic procedures, emergency surgeries she performs on hand trauma patients keep her surgical skills at the highest level. “My work in cosmetics and trauma complement one another,” Dr. Vu acknowledges. “Trauma keeps me attuned to the minute level of detail that will restore function to an injured hand, while cosmetic work requires an aesthetic, almost artistic, appreciation of how the body works.”

Dr. Vu offers a full range of plastic surgical procedures which may include but is not limited to breast augmentation, body contour surgery like tummy tuck, liposuction, or body lifts, as well as facial rejuvenation including face lift, brow lift, rhinoplasty, and eyelid surgery. Dr. Vu is also interested in pursuing areas of post-bariatric surgery and surgeries for men. CREDENTIALS: MAKING SENSE OF CERTIFICATION
Almost everyone has heard that they should look for “board certification” when seeking a qualified plastic surgeon (or any physician for that matter). Most people don’t really understand exactly what this means and what is involved in becoming “board certified.”

Just because a doctor is “board certified” does not mean they are certified by the American Board of Plastic Surgery. Most states can perform cosmetic procedures without being certified in plastics surgery. This means other doctors such as dermatologists, gynecologists and other doctors are performing plastic surgery without having undergone proper training and certifications.

Thus, when choosing a plastic surgeon, it is important to know whether or not the surgeon is certified by the American Board of Plastic Surgery. The ABPS is the only board that certifies physicians in plastic surgery of the face and all areas of the body.

Dr. Kim-Chi Vu is certified by the American Board of Plastic Surgery. In order for her to be board certified by the ABPS, she had to graduate from an accredited medical school, did her internship and residency in general surgery for five years, then completed two years of approved residency in plastic and reconstructive surgery, practiced for a minimum of two years after graduation, then passed an extensive written and oral board examinations which include a review of all the cases she performed in the past year by the members of ABPS.

Dr Vu is also a member of the American Society of Plastic Surgeons, where Dr Vu is required to attend continuing medical education courses and meetings regularly and to adhere to strict code of ethics in her practice as a plastic surgeon.

Even fewer people know about certification of operating facilities, who certifies them and what protection such certification offers. We believe patients need this information to make informed choices in their search for the right surgeon, surgery facility and anesthesia provider.

The American Board of Medical Specialties (ABMS) is the agency which oversees subspecialty boards. More than 100 “boards” have been submitted to the ABMS for formal approval, but only 24 have met their strict educational and examination criteria. They are as follows:

The American Board of Plastic Surgery is the only ABMS board that has traditionally overseen the training and certification of cosmetic and reconstructive surgeons.

You may call the ABMS at 1-866-275-2267 or visit their website at www.plasticsurgery.org to see if your surgeon is certified by the American Board of Plastic Surgery. Give them the name of your surgeon and they will tell you if and when he or she was certified. Certification by other boards does not give you the same protection. Other boards have less strict criteria for certification and some require only a fee. A few of the boards which have not been approved by the ABMS are as follows:
